Please Bring in the following to 198 Geneva Ave. All of your children's birth certificates. Social Security Card for all family members. Medical insurence cards for all family members. Two recent proofs of address (phone bill, utilitybill,etc.) Proff of income (4 recent pay stubs, child support, SSI,welfare letter of recieved TAFDC within past year, foster care benifits, unemployment,etc._
